We are a dedicated and patient-focused Home Health agency seeking a compassionate Home Health Nurse Office Case Manager to join our expanding team. In this role, you will be responsible for assessing, diagnosing, and treating assigned patients while collaborating closely with our field Nurses.

REQUIREMENTS:

New Grads Welcome
Valid nursing license in CA
Excellent customer service and interpersonal skills
Strong problem-solving and critical thinking abilities
Ability to thrive in a fast-paced environment
Proficiency in computer skills

Collaborate with Field Nurses to assess, treat, and diagnose patients
Monitor and report changes in patient symptoms or behavior
Communicate effectively with hospitals, physicians, or specialists regarding patient care
Maintain accurate and up-to-date patient medical records
